List of documents for obtaining an old-style foreign passport for adults

  • Application form for the issuance of an old-style international passport in two copies;
  • Attachment to the application for a passport - Information on work activity for the last 10 years. To be filled out only if information about work activity over the last 10 years does not fit into clause 14 of the application for an old-style foreign passport. The application indicates the remaining data on labor activity;
  • Passport of a citizen of the Russian Federation, identifying the identity of a citizen of the Russian Federation on the territory of the Russian Federation;
  • Passport of a citizen of the Russian Federation, identifying the identity of a citizen of the Russian Federation outside the territory of the Russian Federation - for citizens of the Russian Federation living outside the territory of the Russian Federation;
  • Receipt for payment of state duty;
  • Photo 3 pcs.;
  • Males aged 18 to 27 years living on the territory of the Russian Federation: those who have completed military conscription service or alternative civilian service present a military ID with a mark of completion of conscription military service; those recognized as unfit or partially fit for military service due to health reasons present a military ID with o or “limitedly fit for military service”; those who have not completed conscription military service or alternative civilian service (except for cases where the applicant is declared unfit or partially fit for military service due to health reasons) may optionally submit a certificate from the military commissariat stating that on the day of submitting the application they were not called up for military service or are not sent to alternative civilian service.
  • Command permission, issued in the manner established by the Government of the Russian Federation (only for military personnel of the Armed Forces of the Russian Federation, as well as federal executive authorities that provide for military service, with the exception of persons undergoing military service upon conscription);
  • For urgent consideration of the application (within three working days), one of the following documents is submitted: a letter from the health authority confirming the need for an urgent trip for treatment outside the territory of the Russian Federation; a letter from a foreign medical organization about the need for emergency treatment (at the same time, a medical report from the medical institution at the patient’s place of residence is submitted); a telegraph message received from outside the territory of the Russian Federation and certified in accordance with the legislation of the state of residence (stay), confirming the fact of a serious illness or death of a close relative.
  • Previously issued passport, if its validity has not expired.

Official documents issued on the territory of foreign states, copies of which are submitted for issuing a passport, must be legalized at diplomatic missions or consular offices of the Russian Federation or an apostille must be affixed to the document, unless otherwise provided by international treaties of the Russian Federation.

Documents drawn up in a foreign language must be translated into Russian. The accuracy of the translation and the authenticity of the translator's signature must be notarized.

General recommendations for filling out the application

The standard form of the application form for obtaining a foreign passport is approved by the Administrative Regulations (Order No. 864 of the Ministry of Internal Affairs of the Russian Federation dated November 16, 2017).

The application form consists of two pages and is available for download. The first page contains the child’s details, the second – the legal representative (one of the parents, guardian, trustee, etc.).

The fields “Case No.” and the name of the body that accepted the application in the upper left corner of the form are left empty; they will be filled in by the representative of the government agency that accepted the application.

Further point by point:

  • The child's last name, first name and patronymic are indicated in full.
  • An “X” in the “M” or “F” boxes indicates the gender of the minor.
  • Date of birth is indicated in the format DD.MM.YYYY.
  • The place of birth is written in the same way as on a birth certificate or civil passport.
  • This item reflects data on changes in the child’s last name, first name or patronymic.
    If there are none, the column must be left empty. If changes have taken place, then it is necessary to fully indicate the previous data, as well as the date and place of the change. In the event of a repeated change of name, the application indicates the surname, first name and patronymic at birth, and all subsequent changes are reflected in chronological order in Appendix No. 1 to the application, which is downloaded separately.
  • The permanent place and date of registration are indicated in the format DD.MM.YYYY.

    If the child does not live at the permanent address, then paragraph 7 indicates the place of temporary registration and its duration.

    An application for a foreign passport is submitted to the territorial office of the Main Directorate for Migration Affairs of the Ministry of Internal Affairs, to which this address belongs.

  • The details of the document confirming the identity of a citizen of the Russian Federation are indicated. For a child over 14 years old - this is a general passport.
  • It is necessary to note whether this is the initial receipt of a passport or a replacement for the current one.
  • This item must be completed if the child already has a foreign passport. If there is none or its validity period has expired, then a note is made that there are no other valid passports.

All subsequent paragraphs contain information about the legal representative and are filled out in the same way, except for paragraphs 17 and 18, which optionally indicate a telephone number and email address.

If another legal representative is present when receiving the passport, then a mark indicating agreement with this is made and his details are indicated in paragraphs 21-24.

The signatures of the child and the legal representative are affixed in the presence of a representative of the Main Department of Migration Affairs of the Ministry of Internal Affairs.

Checking the readiness of a foreign passport

The readiness of a foreign passport is checked using the number of the general passport. For old-style foreign passports with a validity period of 5 years (without a chip), it is also possible to check using the number of the birth certificate or other document. Instead of the FMS websites (fmsmoscow.ru, etc.), checking the readiness of a foreign passport is carried out on the official website of the Main Directorate for Migration Issues of the Ministry of Internal Affairs of Russia: Checking the readiness of a foreign passport To check, enter the series and number of the document that you presented when applying for a new foreign passport. Readiness testing can be carried out for residents of both Moscow and other regions.

Instructions for filling

In paragraph 1.
Last name, first name, patronymic are clearly indicated.
In step 2.
Indicate the gender of the child by placing a cross (X) in the appropriate box.

In paragraph 3.

The date, month and year of birth are indicated.

In paragraph 4.

The place of birth is indicated exactly as in the birth certificate (children under 14 years old) or as in the Russian passport (children 14-18 years old).

In paragraph 5.

If the child previously had other last names, first names or patronymics, they should be indicated in full, until what year and the city of their change. If the child has not changed his full name, then the answer in this column is “No.” This column must be filled in regardless of the child’s gender.

If the full name has been changed more than once, then indicate the full name at birth in the questionnaire, and the subsequent changed data is reflected in the appendix to questionnaire No. 2 (downloaded: 28861 times), in chronological order.

In paragraph 6.

Place of permanent residence (registration) of the child: indicate the region (region, territory), locality (city), street, house, building, apartment, date of registration.

In paragraph 7.

The place of actual residence is indicated (for residents of regions who want to obtain a passport at the place of actual residence in another region).

In paragraph 8.

You must provide passport information (children 14-18 years old) or birth certificate information (children under 14 years old).

In paragraph 9.

It is necessary to indicate the details of your previous foreign passport (if any).

The signature of a legal representative, as well as a minor citizen from 14 to 18 years of age, is not placed on the forms; this will happen in the presence of an inspector at the Federal Migration Service.
