Application form for a Schengen visa: rules for filling out

The application form for a Schengen visa consists of 37 points, each of which is subject to a response from the visa applicant. There are two ways to fill out the form:

  1. Online on the website of the consulate or visa center;
  2. By hand on a printed form.

When filling out the form with a pen, it is better to use black ink. Some consulate websites indicate the preferred option of these two, others leave it at the discretion of the visa applicant.

You can fill out the form in English or the language of the country of the consulate. Personally, I always choose English, especially since most of the answers to questions will simply be transliterations. Some consulates allow the use of Russian, resorting to transliteration when writing. This is especially important in the first seven points and 31-32 points. For correct transliteration, use the table below.

The basic rules for filling out are as follows:

  • In each unfilled field, a dash is placed or “does not apply\no (does not apply to you)” is indicated.
  • Only block letters may be used.
  • Nothing is written in the column on the right; this field is intended for the consular officer.
  • It is better to paste the photo before applying for a visa.
  • The application form must be printed on both sides.

Some visa centers offer an additional service of filling out an application form. But why overpay when you can prepare this document yourself using the instructions below.

Filling procedure

The application form for obtaining a Schengen visa consists of 37 questions and each of them must be answered according to individual rules. It’s easy to get confused in them, which is why Russians visiting Europe for the first time prefer to use the help of qualified agencies.

We have prepared a table that can be used as an example of filling out an application for a Schengen visa, but keep in mind that many of the details of the application are different for each individual case.

17Home address and emailYou must enter your actual residential address, even if it differs from your place of registration. In the latter case, you will have to attach a copy of the lease agreement or a certificate from work to the package of papers to confirm that you actually live or live and work at this address. The street name should be indicated using transliteration; words cannot be translated.
18Host countryCitizens of the Russian Federation write “No”, since the state in which they arrive is no different from the one in which they received citizenship.
19Professional activityThe position corresponding to the one indicated in the certificate of employment. Pensioners write PENSIONER, students - STUDENT, and housewives - DOMOHOZYAYKA. If the Schengen visa application form is filled out in English, German or another language, then the position is indicated in it.
20Employer and his address and telephone numberStudents write the name and address of their educational institution. Abbreviations and names must be transliterated. There is no need to translate them.

Instructions for filling out the Schengen visa application form

Let's go through all the points that appear in the questionnaire and find out how to correctly submit information in them.

  1. Last name – write your last name in Latin letters in accordance with your passport
  2. Surname at birth (previous surname) – indicate your maiden name, if there is one, or put a dash (leave it blank).
  3. Name – write your full name in Latin letters in exact accordance with the passport. There is no need to write a middle name.
  4. Date of birth – date format may vary. The most common type is 07/23/1987.
  5. Place of birth - write the city or town in which you were born. At the same time, the abbreviations “g.” or "horizontal" are not indicated. The region is abbreviated as OBL, the republic as RESP. For example, Kazan - Kazan, Sverdlovsk region. – Sverdlovskaya obl, Kazakh SSR – Kazakhskaya SSR, etc.
  6. Country of birth - this item indicates exactly the country in which you were born. If you were born in the USSR, then you should write USSR.
  7. Citizenship is currently Russian Federation. In the subparagraph, citizenship at birth, if different, is indicated by USSR.
  8. Gender – check the box next to the option you want.
  9. Family status. Choose one of the proposed options.
  • If you have never been married - “single”. It is chosen before the first marriage, as well as for children.
  • If you are currently married and living together - “married”.
  • If you are married but live separately - “I do not live with my spouse.”
  • If you are divorced - “divorced”.
  • If the spouse died - “widower/widow”.
  • If you have a same-sex partnership (officially registered) - a “registered partnership”.
  • If you are in a civil marriage and are not officially married, then there is no need.

10. This paragraph is only suitable for minors: last name, first name, address (if different from the address of the applicant) and citizenship of the person with the authority of the parents / legal representative - this paragraph indicates the details of the parents or official guardians of the child. Adult applicants write “not applicable.”

11. Identification number, if available. Russians do not fill out this item, but citizens of other countries may well have it.

12. Type of travel document – ​​o.

13. Travel document number – indicate the number and series of the international passport.

14. Date of issue of the passport - everything is clear with this point, we indicate according to the data of the international passport.

15. Valid until – indicate until what date the passport is valid.

16. Issued by – there are two options here. First, we indicate the transliteration of data from the passport. For example, FMS 123 - FMS 123. The second option is offered in the example of filling out this form on the website of the visa center of one of the Schengen countries - simply indicate the country that issued the passport. In our case, Russian Federation

17. Home address and email of the applicant - we write the address (city, street, house, apartment) and email in transliteration. This paragraph indicates the address of actual residence, not the registered address. Please note that the street name is not translated. That is, if you live on Yablochnaya Street, then it is correct to write Yablochnaya street, not apple street. Below in the special column we indicate the phone number. Next to it is a cell for a phone number. Enter your mobile number there.

18. Country of residence, if not the country of citizenship – select no (No) if you are a citizen of Russia and live in Russia. If you have citizenship of another country, and you live in Russia, then choose yes. In the latter case, you must also indicate the residence permit number and its validity period.

19. Current professional activity - in this paragraph, indicate the name of your position. Write briefly, do not go into details. If your full title sounds like transaction support manager for the commercial directorate, there is no need to translate it all. Just write manager. Pensioners indicate - pensioner, housewives - housewife, freelancers - freelancer. Students should write student.

If you are unemployed, then you need to attach a bank statement with a decent amount of money and a sponsorship letter to reinforce the confidence of the consulate staff in your reliability. An individual entrepreneur in this paragraph writes Individual businessman.

20. Employer, employer's address and telephone number. The abbreviations of the enterprise (LLC, CJSC, JSC) where you work should be translated into English, but the name is simply transliterated. For example, Yabloko LLC will look like LLS Yabloko. JSC is translated as PLS, JSC - CJSC.

21. The main purpose of the trip. Check the box next to the main purpose of your trip. If you go to travel, then this is tourism. For other purposes, select the appropriate option.

22. Country of destination. At this point, write the country where you are going and the consulate to which you are submitting documents. For example, you are going to Spain and apply for a visa to Spain. Then write Spain at this point. And if you submit documents to the consulate of another country (in some regions this is possible), then you still indicate the country of destination. For example, if in Yekaterinburg you submit documents to the Hungarian consulate to obtain a visa to Austria, then your destination country is Austria.

23. Country of first entry. This is the country where you cross the Schengen zone. For example, if you are driving to Germany, you enter the Schengen zone through Poland. We indicate it in this paragraph. Or, for example, you are flying to Spain with a transfer in Riga, then the country of first entry is Riga. The best place to search for air tickets is on Aviasales.

24. Visa requested for. This question indicates what type of visa you want - single, double or multiple entry. I always choose multiple. After all, you need to ask for more, and then see how it turns out. In practice, multiple entry has never been denied.

25. Duration of stay or transit. Indicate how many days your trip will last in the Schengen area. If you are traveling by car to Germany, where you plan to spend 7 days and it will take you about 4 more days to travel there and back. Thus, you will stay in Schengen for approximately 11 days. Write this number in this paragraph. Do not forget that you must have medical insurance for the number of specified days.

26. Schengen visas issued in the last three years. If you have received Schengen visas over the past three years, you must register them in this paragraph. The form requires writing the latest visa, but below you can write in by hand also previously issued ones.

27. Fingerprints previously provided when applying for a Schengen visa. Since 2015, biometrics have been introduced to obtain a Schengen visa. If you have previously submitted fingerprints, please mark yes and write when.

28. Permission to enter the country of final destination, if necessary. This item must be completed only when receiving a visa for transit to a third country.

29. Estimated date of entry into the Schengen area. Enter the date from which you want your visa to begin. Please note that you will not be able to cross the border before this date if the consulate takes into account your wishes and includes it in your visa.

30. Estimated date of departure from the Schengen area. Planned date of end of trip (visa validity).

31. Last name/s, first name(s) of the person inviting you to the Schengen state/s. If this is not available, the name of the hotel(s) or address(es) of temporary stay on the territory of the member states of the Schengen Agreement.

If you are traveling to Schengen countries by invitation, then indicate the contact details of the person who invited you. If you plan to stay in a hotel, then write the name of the hotel and its address in accordance with the reservation. 32. Name and address of the inviting company. To be completed in case of a business trip. For tourism, indicate “not applicable.”

33. The applicant pays for travel expenses during his stay. If you pay yourself, then check the “applicant himself” checkbox and select a payment method (most often it is cash, credit card, etc.). If a visa application form is being filled out for a child or a person who does not have funds to support the trip, then the name and surname of the sponsor is entered in the right column.

34. Personal data of a family member who is a citizen of the European Union, European Economic Area or Switzerland. This item does not apply to a tourist visa, please indicate so or put a dash through. It is filled out only if there are close relatives of EU citizens. According to the Directive of the European Parliament and of the Council on the right of Union citizens and members of their families to move and reside freely in the territory of EU Member States, close relatives have the right to free movement. Close relatives are considered: spouse; son/daughter (under 21 years of age or dependent), as well as direct descendants of the spouse, dependent relatives in the direct ascending line, and similar relatives of the spouse or partner. Brothers and sisters are not close relatives.

35. Relationship with a citizen of the European Union, European Economic Area or Switzerland. Similar to paragraph 34. For tourists – note does not apply.

36. Place and date. Indicate the city and date of filling out the form

37. Signature (for minors - the signature of a person with the authority of a parent or legal representative). A signature is added.

Don't skip the rest of the signature fields. There are a total of four signatures on the form.

How to fill out an application for a Finnish visa?

You can't put dashes! If the answer is negative, the word “no” is written or a tick is placed in front of the “no” item.

You can leave blank only those items that do not apply to you, for example, item 10.

To be sure:

  • clauses 1 and 3 are filled out exactly as in your passport;
  • point 2 – women indicate their maiden name (men and all those who have not changed their last name write the word “no”, no dashes);
  • step 4 – write the date of birth in numbers in the order “day, month, year”;
  • clause 5 and clause 6 – we write the information about the place of birth as it is written in your passport;
  • item 7 – indicate citizenship – Russia or Russian Federation;
  • item 10 – the item is filled out only in the child’s application form (adults leave the item blank, without dashes);
  • item 12 – check the box next to the word “foreign passport”;
  • paragraphs 13-16 – enter the details of a valid international passport;
  • clause 17 – by home address we mean the address of actual residence during a given period (indicate your home phone number, mobile number if desired);
  • clause 18 - oh, if you are a Russian citizen and live in Russia;
  • paragraphs 19 and 20 – indicate real work data and telephone numbers (those not working write “not working”, do not put a dash);

Look in the dictionary to see how your position/profession is written in English and enter it in the appropriate column. If the application is submitted for a student or student, we write “student” or “scholar”.

  • item 22 – indicate the countries you are going to travel to (Finland and then other countries separated by commas);
  • item 23 – indicate the country through the border with which the first entry into the territory of the Schengen zone will be made (usually Finland);
  • item 25 – indicate the period for which you are applying for a visa and the number of days for which you will be able to travel abroad during the validity period of the visa;

For example, 90 days/1 year (90 days/one year) or 30 days/half-year (30 days/half a year). If you are getting a Finnish visa for the first time, most likely you will only be given it for six months.

  • item 26 – here we indicate the dates of the last Schengen visa in your passport (it is enough to indicate it, no need to try to squeeze everything in there);

Visas to Egypt, Thailand and other countries not included in Schengen do not need to be specified. If there were no Schengen visas yet, oh, we don’t write anything where the dates are.

  • item 27 – if you did not submit your fingerprints at the consulate of the Schengen countries for a biometric passport, put “no”;
  • item 28 – we fill it out only if you are asking for a transit visa (does not apply to you – we leave the item blank);
  • paragraphs 29-30 – in the first field we put the date from which you are asking for a visa (usually 8-10 days later than the date of submission of the application form);

End date: if you are asking for a visa for a year, then one year minus one day, if for six months, then six months minus one day.

  • item 31 – indicate the person who invited you, or the place of temporary stay in Finland (most often, this is the address of a hotel or campsite);
  • item 32 – to be filled out if you are traveling for work (if you are a tourist, not to be filled out);
  • item 33 – indicate information about expenses during the trip. If you pay the expenses yourself, check the box next to “Applicant”, and in the column below, mark the necessary items. The items “accommodation paid” and “transport paid” should be noted only if you have confirmation of prepayment of the hotel reservation and transfer (and you can present it);
  • clauses 34-35 – remain empty if you are not a member of the families of citizens of the EU, EEA, Switzerland;
  • item 36 – set a date that will coincide with the date of submission of the application form to the consulate;
  • clause 37 – your signature.

Another signature and date is placed at the very end of the questionnaire! There must be four signatures in total, which you will put on the last sheet of the questionnaire. You can see an example

An application that is missing a signature, or that was signed by someone other than the applicant, is grounds for refusal to issue a visa.

You can apply for a Schengen visa on the website of the embassy or visa center of the country where you are going. The questionnaire forms differ slightly from each other: the flag of the country on the title page, the language of footnotes and duplication of paragraphs. The rules for filling out forms are the same for all Schengen countries.

Sample filling

Filling out the visa application is easy. But people who are faced with this procedure for the first time experience some fear due to the fact that the form may be filled out incorrectly. An incorrectly completed application form may serve as a reason for refusing to accept papers for an entry permit, but it is difficult to make a mistake. In any case, if a tourist doubts which fields need to be filled in and which ones should be left blank, or what information needs to be indicated in a particular paragraph, then he can ask an employee of the visa center or embassy.

Therefore, before this procedure, you should first familiarize yourself with a sample of the correct application. Samples can be seen on general information stands at visa centers or embassies. If the form is filled out online or downloaded from the embassy’s website, then a sample form for filling out the form can be found directly on the same portal.

An example of filling out a form for obtaining a Schengen visa:

  • In paragraph 1 you should indicate your last name. The data should be carefully copied from the baby’s passport. Information is entered in capital Latin letters. Example: IVANOV.
  • Column 2 is not so often filled in for a child visa; this is more relevant for adults. But if the child’s surname has been changed, then paragraph 2 indicates the one that was given at birth.
  • The baby's name is written on line 3. Also, information is taken from the passport to avoid errors.
  • In line 4 the date when the child was born is written. All data is indicated in the following sequence: day, month, year. The date should be written in the same format as indicated in the title of the paragraph - separated by a dash.

  • In line 5 you will need to indicate the baby’s place of birth. The place of birth means the city. The data is also entered in Latin letters. Do not confuse English with Latin letters. All data is entered without translation. Example: Rostov-na-donu.
  • In column 6, enter the country where the baby was born. But if the application form is being filled out for an adult, then it is worth considering that if the applicant was born in a country that was previously part of the USSR, then he should indicate not the country, but the “USSR” union.
  • Point 7 indicates the child’s nationality at the time of filling out the form. There are two sub-clauses – current citizenship and citizenship at birth. And if the child’s citizenship at birth is different, then it is worth indicating this point.
  • In line 8 the gender of the baby is noted. Information is indicated by placing a cross in the already prepared marks. On visa forms, the female gender is indicated as “Female” and the male gender as “Male”.
  • In line 9 you should indicate your marital status. For children, select the “Single” option, which means “not married”.
  • Section 10 indicates the personal data of the baby’s parents: first names, last names and nationalities. If the address of registration or residence differs from the address of the baby, then it should also be indicated.
  • Line 11 indicates the TIN (if any). This column is not required to be filled out. It is worth noting that TINs are not assigned on the territory of the Russian Federation. Therefore, you can put a dash in this field.

  • Section 12 indicates the type of travel document on which the baby will travel. For children it is set o.
  • Column 13 records the travel document data: series and number.
  • In paragraph 14, you should write down the date of issue of the travel document.
  • Column 15 indicates the expiration date of the travel document.
  • Paragraph 16 provides detailed information regarding the authority that issued the travel document.
  • The address is entered in column 17. It is worth remembering that the address should also not be translated into another language. The street is written in Latin letters. Example: LENINA 45, KV. 28.

You should also write down your phone number and email address, if available, in this line.

Sample of filling out the second page of the visa application form

  • In line 18 it is worth answering whether the child has dual citizenship with another country or not.
  • In column 19 of the child’s application form, you should indicate “no profession.” This line usually indicates the position in which the applicant works.
  • On line 20 you will need to indicate the address of the educational institution: university, school or preschool. If the child is already a student at the time of filling out the form, then he must additionally indicate the telephone number of the educational institution.
  • In column 21, you should select one of the items that highlight the purpose of the trip.

Often, when filling out this column of the questionnaire, the child chooses tourism or visiting relatives.

  • Column 22 lists all EU countries where the baby plans to visit with his parents. It is worth indicating even transit countries.
  • The state of entry into the Schengen territory is entered in line 23. This means that you need to enter exactly the state where the child and his parents will cross the borders of the Schengen zone for the first time.
  • Line 24 indicates the number of crossings of Schengen borders. The desired visa is also indicated: single-entry, multiple-entry or double-entry.
  • Column 25 indicates the exact number of days spent in the territory of the EU countries.
  • In line 26 you should indicate all previously issued entry permits. You should provide information about visas in this order: Portugal, from 25-10-2007 to 25-01-2008. Spain, from 25-08-2009 to 15-11-2010. It is worth remembering that only visas issued over the last three years are indicated. If a person did not have open visas, then it is worth putting about.

Fingerprinting procedure

  • In line 27 it is worth answering whether the child underwent a fingerprinting procedure. Let us remind you that from the age of 12, fingerprinting is mandatory. Young children may submit fingerprints only with parental permission.
  • Column 28 is filled in if a transit visa is required.
  • Point 29 indicates the planned date of entry into the Schengen territory.
  • Column 30 indicates the planned departure date.
  • In paragraph 31, you should indicate information about the person being invited or the address of the hotel complex where the child and his parents plan to stay during the trip.
  • Line 32 is not filled in. You need to put a dash in it.

Sample of filling out the third page of the visa application form

  • In paragraph 33 they indicate at whose expense the expenses associated with the trip will be incurred. This is one of the main questions when filling out a child’s questionnaire. The sample on our website shows the exact translation of all possible options.
  • On line 36, enter the city where the form was filled out. Example: Rostov-na-donu 10-07-2021.
  • Line 37 is marked.

The electronic form is no different from the paper form. But in electronic form, some items are written differently, namely in columns 4 and 14 the date is indicated in the format “year, month, day.”
