By hand

This method is suitable for people with neat and legible handwriting. You must write in block letters; the inspectors must understand every word, otherwise they may refuse. You can use a pen with black or blue ink. Correcting, crossing out, or painting over anything with a proofreader is strictly prohibited. If you made a mistake, fill out a new form.

On your computer in MS Word or PDF

Paragraph 1

Write your last name exactly as in your passport, in Latin characters. If it is double, do not forget to put a hyphen between its parts.

Please also print your name in Latin letters; look it up in your foreign passport to do it correctly.

Pay attention to this field in the example of filling out the application form for a Spanish visa in Spain. Only the name of the city, village, town is entered here, for example, YAROSLAVL, VELIKOYE, PERVOMAYSKY, region and district - no need. Do not add the words “city”, “settlement”, “village” and their abbreviations, such as “city”, “city”, “settlement”, “p.”, “village”, “s.”

Sometimes the department that issued the document is written here, for example, the Ministry of Internal Affairs. This is incorrect, you need to write the country, in our case - RUSSIAN FEDERATION.

Clause 17

Enter the address here without any additions such as “street”, “avenue”, “house” or their abbreviated versions. Only the name in the form of a transliteration (TVERSKAYA), then after a space - a house, after a hyphen - an apartment and next to it in parentheses - a city (in English or Spanish).

If you have a residence permit or citizenship of another country, write its number and expiration date. If they are missing, then Fr.

Please indicate the full name of the institution where you work or study. If it does not have an official English translation, type it in translit form, replacing LLC with LLC, AO with JS. Point 21

The type of visa depends on the purpose of the trip. Sometimes mistakes are made here: o or “Sport” when going to a match or concert as a spectator. This is incorrect, in this case a mark should be placed next to the word “Tourism”, and the previous ones are suitable only for athletes or artists and are confirmed by a written invitation from the host organization.

A Spanish treatment center may invite you to provide medical services, in which case select “Treatment”. To travel to relatives or friends, you must check the appropriate box and support it with an invitation letter.

If you have to make a transfer there, then select “Transit through the airport”, and if you are visiting it while passing through, then simply select “Transit”.

“Other” is marked if you are participating in a conference and under other circumstances.

Pay special attention, at this stage the most errors occur. You must indicate the country where you will first cross the Schengen zone. That is, if you fly to Spain and change trains in Germany, then you need to enter it on the form (GERMANY). But if you make a transfer on the territory of a state that is not part of Schengen, for example, in Belarus, then for the first time you will enter its territory directly on Spanish soil. Then you need to write it, then the answers in points 22 and 23 will be the same - SPAIN.

Point 24

If the company that sent the invitation pays for you, then the first two positions in the right field are noted, as well as what it will pay for: transportation, accommodation, etc.

Those who have relatives living in the European Union, European Economic Area or Switzerland must indicate their personal data in the appropriate fields: surname, first name, date of birth, citizenship, passport number or other identification document.

Paragraph 35

Here you need to put a tick next to the item indicating the degree of relationship with him. This applies to spouses, daughters/sons under twenty-one years of age, direct descendants of the spouse, relatives in the direct ascending line, adoptive parents, adopted children, guardians, brothers/sisters (blood, full and half-blood), etc.

Paragraph 36

Signatures

The form must be printed in A4 format on both sides of two sheets and signed in four places:

  • in window 37;
  • after being notified that the documentation package will not be returned if you are refused;
  • after information about compulsory health insurance for visiting EU countries;
  • near the date on the last page.

  • A passport valid for at least three months after the last expected date of departure from the Schengen area, containing at least two blank pages and not older than ten years;

  • A copy of all pages of other international passports: valid and canceled;

  • A copy of the birth certificate - for minors;

  • Economic guarantees: Certificate of employment indicating position and salary. Applicants who have their own business provide a copy of the business registration certificate and a copy of the Individual Taxpayer Number (TIN) document;
  • Financial guarantees: €64.53 per person for each day of stay (minimum €580.77 per trip per person). Proof may be a bank account statement on bank letterhead, traveler's checks or a certificate of currency purchase, a credit card accompanied by a summary of the bank account;
  • For minors traveling unaccompanied by parents or guardians: a copy of a notarized travel permit from both parents/guardians or one parent/guardian who is not accompanying the minor child.
