Requirements for the Schengen visa application form

Is it possible to fill out the form online yourself?
The application form can be filled out online on the Visa Application Center website and edited within 90 days from the date of writing. After printing the form, you should write the date of completion and sign.
The form can be filled out in Russian, English, Finnish or Swedish. All letters must correspond to the Latin alphabet.

How to fill out an application for a Finnish visa?
You can't put dashes! If the answer is negative, the word “no” is written or a tick is placed in front of the “no” item.
You can leave blank only those items that do not apply to you, for example, item 10.
To be sure:
- clauses 1 and 3 are filled out exactly as in your passport;
- point 2 – women indicate their maiden name (men and all those who have not changed their last name write the word “no”, no dashes);
- step 4 – write the date of birth in numbers in the order “day, month, year”;
- clause 5 and clause 6 – we write the information about the place of birth as it is written in your passport;
- item 7 – indicate citizenship – Russia or Russian Federation;
- item 10 – the item is filled out only in the child’s application form (adults leave the item blank, without dashes);
- item 12 – check the box next to the word “foreign passport”;
- paragraphs 13-16 – enter the details of a valid international passport;
- clause 17 – by home address we mean the address of actual residence during a given period (indicate your home phone number, mobile number if desired);
- clause 18 - oh, if you are a Russian citizen and live in Russia;
- paragraphs 19 and 20 – indicate real work data and telephone numbers (those not working write “not working”, do not put a dash);
Look in the dictionary to see how your position/profession is written in English and enter it in the appropriate column. If the application is submitted for a student or student, we write “student” or “scholar”.
- item 22 – indicate the countries you are going to travel to (Finland and then other countries separated by commas);
- item 23 – indicate the country through the border with which the first entry into the territory of the Schengen zone will be made (usually Finland);
- item 25 – indicate the period for which you are applying for a visa and the number of days for which you will be able to travel abroad during the validity period of the visa;
For example, 90 days/1 year (90 days/one year) or 30 days/half-year (30 days/half a year). If you are getting a Finnish visa for the first time, most likely you will only be given it for six months.
- item 26 – here we indicate the dates of the last Schengen visa in your passport (it is enough to indicate it, no need to try to squeeze everything in there);
Visas to Egypt, Thailand and other countries not included in Schengen do not need to be specified. If there were no Schengen visas yet, oh, we don’t write anything where the dates are.
- item 27 – if you did not submit your fingerprints at the consulate of the Schengen countries for a biometric passport, put “no”;
- item 28 – we fill it out only if you are asking for a transit visa (does not apply to you – we leave the item blank);
- paragraphs 29-30 – in the first field we put the date from which you are asking for a visa (usually 8-10 days later than the date of submission of the application form);
End date: if you are asking for a visa for a year, then one year minus one day, if for six months, then six months minus one day.
- item 31 – indicate the person who invited you, or the place of temporary stay in Finland (most often, this is the address of a hotel or campsite);
- item 32 – to be filled out if you are traveling for work (if you are a tourist, not to be filled out);
- item 33 – indicate information about expenses during the trip. If you pay the expenses yourself, check the box next to “Applicant”, and in the column below, mark the necessary items. The items “accommodation paid” and “transport paid” should be noted only if you have confirmation of prepayment of the hotel reservation and transfer (and you can present it);
- clauses 34-35 – remain empty if you are not a member of the families of citizens of the EU, EEA, Switzerland;
- item 36 – set a date that will coincide with the date of submission of the application form to the consulate;
- clause 37 – your signature.
Another signature and date is placed at the very end of the questionnaire! There must be four signatures in total, which you will put on the last sheet of the questionnaire. You can see an example here (check the boxes).
An application that is missing a signature, or that was signed by someone other than the applicant, is grounds for refusal to issue a visa.
How to fill out an application for a Schengen visa?

The surname is in the nominative case (example: IVANOVA).
